I will wear the company uniform each day as part of my employment

I understand that sturdy work boots that cover my ankle are to be worn each day; I must have and wear them to work on my first day of employment

I will not use drugs and/or alcohol before or during work hours, and at no time will I bring either onto company property, job sites, and company vehicles

I will under no circumstances bring a gun to work even if I contain a permit to do so

Cigarette smoking is prohibited on the job except on approved breaks

Cell phone use on the job is limited to calls of work nature or emergency only

I am physically fit and able to lift to 80 lbs if necessary

I will treat with great care and respect the tools, equipment, and vehicles of Jetstream

I understand no tools, equipment or trucks are to be borrowed from Jetstream

I understand that Jetstream is an Irrigation firm, not a bank – they do not give loans

I have not signed a non-compete or non-solicitation agreement with another business

Jetstream, in considering my application for employment, may verify the information set forth on this application and obtain additional background information relating to my background. I authorize all persons, schools, companies, corportations, credit burewus and law enforcement agencies to supply any information concerning my background.

I understand that Jetstream has a commitment to maintain an alcohol/drug-free workplace and that Jetstream employees are prohibited from possessing, using, being under the influence of or participating in any transaction involving alcohol, illegal drugs or other mind-altering substances on Jetstream property, in Jetstream vehicles or while on Jetstream business. Jetstream does not permit the use of alcohol or drugs during the workday. I further undertstand and agree that if I am employeed, I may be required to submit to alcohol/drug-testing under certain circumstances during my employment.

I agree that any lawsuit arising out of my employment, my application for employment, or termination from employment with Jetstream must be filed no more than 180 days after the date of the employment action that is the subject of the lawsuit. While I understand that the statute of limitations for claims arising out of an employment action may be longer than 180 days, I agree to be bound by the 180-day period of limitations, and I WAIVE ANY STATUTE OF LIMITATIONS TO THE CONTRARY.
